Nomination Procedure

Application Forms

The Applicant must submit all the duly accomplished forms (except for the Mentor/Adviser and Colleague Assessment forms – Forms 8.1 to 9.3) in an unsealed short brown envelope to the office of the School President (or the school’s designated official).

The Applicant should reproduce Forms 3.0, 4.0, and 5.0 of the Application Form Set as required* . Each applicant is required to submit three (3) Mentor / Adviser Assessments and three (3) Colleague Assessments** . A mentor/adviser who accomplishes the form must be a bona fide member of the school staff or faculty or an organizationally recognized mentor / adviser of a civic or community organization (if the organization is not school-based). He must be officially serving or has previously served as a mentor / adviser for the organization where the applicant is an officer. The Colleague Assessment must be accomplished by a duly enrolled student of the school who has worked with the applicant for at least one school year as fellow member or a co-officer in the school or community-based organization. Accomplished Mentors / Advisers and Colleagues Assessment forms must be submitted in a sealed long letter envelope, with the mentor/adviser or colleague’s signature across the flap, directly to the office of the School President (or the designated official).

The school must check the Application Forms, duly filled out by the Nominee, for accuracy and completeness BEFORE these are sealed and signed for final submission to the AYLC Secretariat.

School Nomination Form

As soon as the final set of the School’s Nominees*** has been identified, the School President (or the designated official) shall accomplish the School Nomination Form.

  1. Name of Nominee. List the Nominees according to order of Percentile ranking in College Batch.

  2. Course and Year graduating from college. Indicate the course that the student is currently taking and the year he/she is expected to graduate from college. (Examples: BS Mechanical Engineering / 2012, BS Nursing / 2012, Bachelor of Fine Arts / 2012)

  3. Academic Standing.
    1. Cumulative Grade Point Average. Compute the student’s cumulative grade point average:
      1. Get the credit points per subject by multiplying the grade points acquired by the corresponding number of units earned,
      2. get the sum of all credit points and
      3. divide this by the sum of all units earned.

    2. Highest Possible Grade Point Average. Indicate the highest possible grade point average applicable in your school. This should be the same for all of your nominees.

    3. Rank in College Batch. Indicate the student’s present academic rank in his/her college batch (Examples: College of Engineering, College of Nursing, College of Architecture and Fine Arts) graduating by the end of school year 2011-2012. Use the nearest fifth percent (i.e. top 5%, top 10%, top 15%, etc.) by putting a check mark on the appropriate box or column per nominee.
  4. Imprint the Nomination Form with the official school dry seal.

True Copy of Grades and Semestral Academic Achievements

In a SEPARATE short brown envelope, enclose all

  1. True Copy of Grades and
  2. a summary of the semestral academic achievements for all of the nominees.

The Registrar should duly authenticate both documents.


* Form 3.0: one copy for every position with a maximum of 10 positions. Form 4.0: one copy for every award with a maximum of 10 awards.

**The same mentor /adviser or colleague may submit assessments for more than one applicant.
***Each school is allowed a maximum of 10 nominees
